Who We Are Community Living Concepts, Inc. (CLC) is a private proprietary corporation that owns and operates seven (7) Intermediate Care Facilities for people with Mental Retardation (ICFs / MR) in and around the Cleburne/Johnson County area of north central Texas. CLC was established in June 1980 as Cleburne Living Center. However in 1981 when CLC was unable to open its first facility in Cleburne, the name was changed to Community Living Concepts to reflect CLC’s involvement in communities other than Cleburne. Jan Hannah is CLC’s sole owner. Who We Serve CLC serves 55 individuals with disabilities ranging from mild to severe in 7 residential locations in Johnson County, TX. To qualify individuals must be 18 years of age or older, have a primary diagnosis of mental retardation or a related condition and be eligible for Medicaid services. CLC maintains a waiting list and when an opening is available, offers services first to individuals in crisis then to individuals whose families live in or near Johnson County. Lastly, individuals who live in other areas of Texas are considered. Every effort is made to match the personalities, interests and activity levels of individuals entering CLC’s services to those living at the facility that has a vacancy. Because we consider the matching of the individual with his/her housemates so critical, each person is asked to visit for three or four days a minimum of one time before placement is discussed. How We're Doing In 1989 one of CLC’s facilities received a zero deficiency survey. That survey started a trend that has continued through 2004. Since 1989 there have been over 60 “Zero D” surveys for the 7 facilities. While one or two deficiency free surveys are not necessarily indicators of good services, it is our belief that over 60 in 7 different locations in just over a decade serves as one indicator of CLC’s dedication to quality services. Other indicators include long tenured staff, stability in the people we serve, and a high degree of satisfaction among the families of the people served as well as the individuals themselves.
Guiding Principles and Philosophy The belief that individuals can consciously direct their own lives is the primary principle upon which Community Living Concepts is founded. We believe each individual has the right to make choices and decisions according to his/her own ability. We believe such decisions and choices should be encouraged and supported. We believe people with mental retardation can benefit from and are entitled to supports and services which will enable them to direct their own lives.
Mission Therefore, it is the mission of Community Living Concepts to support persons with mental retardation in acquiring skills which will allow them to be as independent as possible, achieve acceptance as a valued member of society and enjoy participation in the community of their choice.
